Elder Care Servies Serving Nazareth, KY

The SHIP program provides health insurance information, counseling and assistance to seniors and disabled individuals. This service is provided at no charge by local, well-trained counselors.

SERVICES INCLUDE: Adult Day Care & Alzheimer's Disease Respite program, Caregiver Support Services, Chronic disease self management program, Consumer Direct Option (CDO), Hart- Support Living ( provides funding to disabled Kentuckians to supports them with independence living at home), Homecare program, Personal care program, Senior centers, Employment program, SHIP ( State health insurance), LTC Ombudsman, and Information & Assistance.
