SERVICES INCLUDE: Adult Day Care( person age 60+ who is physically disabled, mentally confused or frail and who needs supervision or assistance during part of the day), CDO program(Medicare Waiver members who are receiving or become eligible to receive Home & Community Based waiver services), Case Management, Homemaker Management, Personal Care, Respite services, LTC Ombudsman,Transportation,SHIP(State Health Insurance), Family Caregiver program, and Information & Assistance.
