The Weld County Area Agency on Aging is a Help Source for Adults 60+. Our mission is to empower older adults, family caregivers, and persons who require assistance in the Medicaid long term care system to live independent, healthy, and dignified lives by providing services and linkages to available community services. PROGRAMS FOR ACTIVE SENIORS: Chore Service Program, Dental, Vision & Hearing Program, Grandparents Raising Grandchildren, Hispanic Elderly Outreach, Information & Assistance, Weld Aging Well (Healthy & Active Living Programs), Legal Assistance, Peer Counseling, Senior Nutrition. SUPPORT SERVICES: Adult Day Care, Family Caregiver Support, Home Delivered Meals & Home Health Care. LONG TERM CARE SOURCES: Options for Long Term Care & Long Term Care Ombudsman.
