Elder Care Servies Serving Portage, WI

The Board on Aging and Long Term Care primarily advocates for people aged 60 and older, who reside in a Nursing Home (NH), Community Based Residential Facility (CBRF), Adult Family Home (AFH) or receive funding through The Community Options Program (COP).

Formerly the Columbia County Aging Unit, the ADRC provides a 'one stop shop' for information and assistance and administers aging programs funded by the Federal Older Americans Act as well as other services including the Elderly Benefit Specialist program.

Aging units in the planning & service area include Adams, Ashland, Barron, Bayfield, Brown, Buffalo, Burnett, Calumet, Chippewa, Clark, Columbia, Crawford, Dodge, Door, Douglas, Dunn, Eau Claire, Florence, Fond Du Lac, Forest, Grant, Green, Green Lake, Iowa, Iron, Jackson, Jefferson, Juneau, Kenosha, Kewaunee, La Crosse, Lafayette, Langlade, Lincoln, Manitowoc, Marathon, Marinette, Marquette, Monroe, Oconto, Oneida, Outagamie, Ozaukee, Pepin, Pierce, Polk, Portage, Price, Racine, Richland, Rock, Rusk, Saint Croix, Sauk, Sawyer, Shawano, Sheboygan, Taylor, Trempealeau, Vernon, Vilas, Walworth, Washburn, Washington, Waukesha, Waupaca, Waushara, Winnebago, Wood. Aging Units of: Bad River, Forest County Potawatomi, Ho-Chunk, Lac Du Flambeau, Lac Courte Oreilles, Menominee County/Tribe, Oneida, Red Cliff, Sokaogon Chippewa, Stockbridge-Munsee, and St Croix Tribes.
